To keep our records up to date, please fill out the following form. Please submit a new form for each member of your family.

There is an option to attach a photo that will be part of your Breeze profiile. Breeze is our church management system and gives members and attenders with an account the ability to search for contact info of other members/attenders, much like a church directory. If you do not have a Breeze account and would like one, click here. Individual pictures are used for each person's account but there is also a place for a family photo, so you can submit both if you'd like. Photos are optional.

Note: If certain information is the same for all members of the family (ex: address) you do not need to enter it for every member, just in the first entry.
